Future Goals™ Teacher of the Month

The NHL and the NHLPA would like to recognize all the great work that you are doing in the classroom to help bring STEM concepts to life for your students through the sport of hockey and want to share that with the public. If you are interested in participating in the Program, please complete and submit the following questionnaire, along with a headshot (please make sure that only you appear in the headshot) (collectively, your “Submission”). We will share your Submission with NHL Interactive CyberEnterprises, LLC and Hockey Ventures (Canada) Inc., the sponsors of the Program, and your local NHL Club, so that they can administer the Program and recognize your efforts in the classroom on the Program website. If you are voted as the Future Goals™ Most Valuable Teacher of the Month you could win an NHL® jersey. Plus, if you’re voted as the Future Goals™ Most Valuable Teacher of the Year, you could win a personal technology device (e.g., phone, laptop, or computer).,

Agree that EverFi may share my Submission, including my personal information, with NHL Interactive CyberEnterprises, LLC and Hockey Ventures (Canada) Inc. (the “Sponsors”), and my local NHL Club, for the purposes of administering the Program.

Agree that the Sponsors and my local NHL Club may post my Submission and related content (excluding my contact information) on the Program website at NHL.com/MVT and that such information, once posted, may be shared by the public through social media and other electronic means.

Agree that, if I am selected as a winner, I may be filmed, photographed, or otherwise recorded, to the extent permitted by applicable law, either in person or via remote access (e.g., Zoom) in connection with the awarding of any prize or otherwise as it relates to the Program.

Agree that the Sponsors and their affiliates, agents and affiliated third parties may use my Submission, name, school, likeness, filmed/recorded content, photograph, video, voice, opinions and/or hometown and state/province, and/or other content for promotional purposes in any media now known or hereafter devised, worldwide, without further payment or consideration, notice, review or approval, including to support, advertise, and/or promote current and future iterations of the Program and the Program’s public fan vote.

Represent and warrant to the Sponsors that my Submission is my original work; that I am not prohibited from participating in the Program (or if I am selected to be a winner, that I am not prohibited from accepting any prize/Donation on behalf of myself and/or my school/district (if applicable)), whether by school policy, contract or otherwise; and that I have obtained the school’s permission to participate in the Program and also obtained the school principal’s permission to share their name and contact information.
