The Future Goals Most Valuable Teacher Program allows us to celebrate our everyday heroes in the classroom, the teachers, who do so much for children every single day. Parents, students, faculty, and administrators can all show support for their favorite teacher by nominating their teacher for this honor.

2025 Future Goals™ Most Valuable Teacher Nomination Form

The National Hockey League (“NHL”) and the National Hockey League Players’ Association (“NHLPA”) would like to recognize all the great work that teachers are doing in the classroom to help bring STEM concepts to life for students through the sport of hockey and want to share that with the public. If you are a teacher and would like to nominate yourself or if you would like to nominate a teacher you would like to see recognized in the Future Goals™ Most Valuable Teacher Program” (the “Program”), please complete and submit this nomination form, along with a headshot of the nominated teacher (the “Nominee”). Please ensure that only the Nominee appears in the headshot without any accompanying trademarks or logos (collectively, your “Submission”). Your submission will be reviewed by a select judging panel comprised of Sponsors’ (as defined below) and EVERFI (the administrator of the NHL and NHLPA Future Goals™ Program) representatives. If your Nominee is selected to represent their local NHL Club Market as a finalist to be put up for public vote for the Most Valuable Teacher of the Year Award, the Nominee will be notified between January 14, 2025 and February 17, 2025. Please note that you must have the permission of the Nominee (i.e., the person you wish to nominate) to be included in your Submission (including permission to use their photograph). Agree that I have permission of the teacher I have nominated and that EVERFI may share my Submission, including my personal information and my Nominee’s information, with NHL Interactive CyberEnterprises, LLC and Hockey Ventures (Canada) Inc. (the “Sponsors”) for the purposes of administering the Program (including but not limited to judging and scoring my Submission), and, if my Nominee is selected as a Finalist and agrees, EverFi may also share my Submission with my Nominee’s local NHL Club in furtherance of administering the Program (including through spotlights, driving public voting, and other Program-related activities).

Agree that, if my Nominee is selected as a Finalist and agrees, the Sponsors may post my Submission and related content (excluding my contact information) on the Program website at NHL.com/MVT and that such information, once posted, may be shared by the public through social media and other electronic means.

Agree that, if I (or the teacher I have nominated) am/is selected as a winner, I/they may be filmed, photographed, or otherwise recorded, to the extent permitted by applicable law, either in person or via remote access (e.g., Zoom) in connection with the awarding of any prize or otherwise as it relates to the Program.

Agree that, if my Nominee is selected as a Finalist and agrees, the Sponsors and their affiliates, agents and affiliated third parties may use my Submission, name, school, likeness, filmed/recorded content, photograph, video, voice, opinions and/or hometown and state/province, and/or other content for promotional purposes in any media now known or hereafter devised, worldwide, without further payment or consideration, notice, review or approval, including to support, advertise, and/or promote current and future iterations of the Program and the Program’s public fan vote.

Represent and warrant to the Sponsors that my Submission is my original work; that I (or the person I have nominated) am/is not prohibited from participating in the Program (or if I/they am/are selected to be the winner, that I/they have the school’s permission to participate, am/are not prohibited from participating in the Program, and if selected as the winner, I/they are not prohibited from accepting any award on behalf of myself/themselves and/or my/their school/district (if applicable)), whether by school policy, contract or otherwise and that I have also obtained the school principal’s permission to share their name and contact information.
